Appeal a denied privacy rights request

If Caremaze has denied your privacy rights request in whole or in part, you have the right to appeal our decision.

When to File an Appeal

You may file an appeal if:

  • We denied your request to access, delete, correct, or port your personal information
  • We denied your opt-out request for sale, sharing, or targeted advertising
  • We denied your request to limit the use of sensitive personal information
  • We denied your request related to automated decision-making
  • You disagree with our verification decision

In your appeal, please include:

  • Your full name and contact information
  • Reference number from your original request (if provided)
  • Date of the original denial
  • Specific reason(s) you believe the denial was incorrect
  • Any supporting documentation

Appeal Process

  • Receipt: We will acknowledge receipt of your appeal
  • Review: We will conduct an independent review of the original decision
  • Response: We will provide a written decision within 45 days of receiving your appeal (with a possible extension of up to 45 additional days as permitted by law)
  • Explanation: If we uphold the denial, we will explain the reasons

If you are not satisfied with the outcome of your appeal, you may have additional rights under your state's privacy law:

  • California (CCPA/CPRA): Contact the California Attorney General at oag.ca.gov
  • Virginia (VCDPA): Contact the Virginia Attorney General at www.oag.state.va.us
  • Colorado (CPA): Contact the Colorado Attorney General at coag.gov
  • Connecticut (CTDPA): Contact the Connecticut Attorney General at portal.ct.gov/AG
  • Other States: Contact your state Attorney General's office or consumer protection agency

If your appeal relates to an automated decision that produced legal or similarly significant effects, we will:

  • Provide information about the logic involved in the decision
  • Explain the significance and consequences of the processing
  • Consider your appeal with human involvement
  • Provide an opportunity to express your point of view
  • Explain our final decision
